What is Targeted Case Management?

A Targeted Case Manager (TCM) helps advocate for their clients' needs. The main focus on Case Management is to help clients get the support they need to succeed. A TCM may provide the following services for a client:

• Find social support to improve socialization skills of clients

• Make referrals for medical, dental, and mental health needs

• Offer educational support

• Provide help with housing, food, clothing, and transportation

• Assists clients with applying for and gaining government benefits

• Create plans and establish goals with clients

• Help clients discover their strengths

• Case Management aims to eliminate barriers that may prevent individuals from meeting social, educational, and employment needs.​                                     ​​   ​
